Job Description
Key Objectives:
Responsible to document the life and progress of each individual child and youth in our Program
Key Responsibilities:
Conducts intake interviews, collateral interviews and eligibility assessment and prepares recommendation report based on preliminary assessment
Prepares and collates the necessary files of the children and youth in his/her charge, and assessment summary; and updates the case study report annually or as required after each care conference
Suggests and participates in the implementation of possible intervention, and facilitates in explaining child’s behavior according to his/her chronological age development stage
Handles supplemental activities for children, Documents progress of a child according to SOS Developmental Framework, accomplishes the medical and developmental assessment of children and youth assigned, and facilitates the presentation and analysis of different data in relation to child’s IDP
Prepares Discharge Summary and After-Care monitoring forms and coordinates with the youth’s biological family
Requirements:
Candidate must possess at least Bachelor's/College Degree in Social Science/Sociology, Psychology or equivalent. Licensed Social Worker
At least 1 Year(s) of working experience in the related field is required for this position.
Required Skill(s): Guidance and Counseling, Case Management, Facilitation, Data Gathering
Preferably Less than 1 year experience specialized in Social & Counselling Service or equivalent.
